Frequently Asked Questions About In-Home Hospice
Referrals can come from a physician but you do not have to wait for your own doctor to refer you to Hospice. You can initiate the conversation regarding a referral to Hospice Care by calling us at 210-475-3731.
First, an in-person assessment will be completed to determine if you are eligible for Hospice Services as defined by Medicare.
If you are eligible and choose Hospice, you will be admitted. We will then arrange for hospice-related medical equipment, supplies and medications to be delivered to you. Your RN Case Manager will coordinate their scheduled visits with you. You will be offered Social Work and Spiritual Services, if you choose to accept these services, you will be visited by our Social Worker and Chaplain within 5 days of admission and you will coordinate their scheduled visits. If you need assistance with things like bathing or dressing, we will arrange for a Hospice Aide to provide these services.
